Admissions Checklist and Forms

Your admissions file will be marked as complete after you have completed the online application and enrollment packets, paid the fees and submitted all the required documents. Documents may be emailed to admissions@desertchristian.com, faxed to 661-948-0858, or delivered to the main office. Students will not be accepted until they have completed any required assessments or interviews.

 

•Kindergarten Applicants: 
Copy of official birth certificate 
Copy of current immunization record 

 

•Elementary (1st Through 5th Grade) Applicants:
Copy of official birth certificate 
Copy of current immunization record 
Full report card for last grade completed
Most recent report card for current year
Latest Standardized Test Scores (2nd-5th grade)
 
•Middle School (6th Through 8th Grade) Applicants:
Copy of official birth certificate 
Copy of current immunization record (Incoming 7th grader must have a T-Dap Booster)
Full report card for last grade completed
Most recent report card for current year
Latest Standardized Test Scores 
 
•High School (9th Through 12th Grade) Applicants:
Copy of official birth certificate 
Copy of current immunization record 
Full report card for last grade completed 
Latest Report Card (9th grade)
Transcript (10th - 12th grade) 
Latest Standardized Test Scores 
  • All incoming kindergarten and first grade students will be screened to determine the best academic placement for each student. These in-person screenings will be scheduled once the student's file is completed.
  • Elementary and Middle School assessments and family meetings will be scheduled once the student's file is completed.
  • High School student interviews with the principal will be scheduled once the student's file is completed.
